Job Summary

The Role

The Community Clinical Nurse Specialist will:

The Community Clinical Nurse Specialist in Palliative Care leads on the assessment, planning and delivery of a consistently high standard of person centred specialist palliative care. They lead on the coordination and day‑to‑day management of the service to ensure a consistently high standard of service provision and develop and maintain good working relationships with the multidisciplinary team internally and externally to the organisation.

The role supports service developments, acts in accordance with the NMC Code of Conduct, and works in line with the organisation’s values and clinical strategy. The Community Clinical Nurse Specialist demonstrates the core competencies of the CNS, which include Clinical, Education, Leadership, and Audit and Research.

Requirements

The Community Clinical Nurse Specialist will be able to demonstrate:

  • 1st Level Registration, evidence of personal and professional development, and experience of working at Band 6 or equivalent.
  • Experience of working and studying in specialist palliative care, leadership, change management, and working in the community.
  • Non‑medical prescriber, Advanced Communication skills course, and mentorship and teaching experience.
  • Ability to play an active role in education, work safely and autonomously as a lone worker and as part of a team, and evaluate research to ensure evidence‑based practice.
  • A good understanding of clinical governance and its application in practice, ability to establish good working relationships to promote best practice and take a lead in change management.
  • Experience and understanding of current issues in specialist palliative care community nursing, and ability to work effectively with a wide range of stakeholders in the community.
  • We are also seeking someone who can analyse, interpret, collate and utilise a wide range of information to enhance service development, demonstrate the ability to set and establish priorities, manage time, meet deadlines, and communicate effectively across a range of services.
  • You will bring competent IT skills, audit experience, strategic competence, and the ability to take on board new concepts and problem solve.
  • Personal attributes include the ability to manage the emotional impact of working in a specialist palliative care team, promote cohesive and effective team working, be self‑aware, and be responsible for your own CPD and personal development.

The Role

The Advanced Nurse Specialist ensures accurate patient documentation and effective communication with the multi‑disciplinary team. They oversee junior colleagues’ development, contribute to an effective learning environment, and lead on strategic and organisational education and service development.

Responsibilities include recording, analysing and reporting data, supporting clinical governance, and acting in line with the hospice values and clinical strategy.

The role provides leadership within the 7‑day community service and urgent response service, demonstrates advanced holistic assessment skills, enables sensitive discussions with patients and families, acts as a specialist resource for the MDT, and supervises cluster‑based working.

The Person

We’re looking for a highly experienced professional who can demonstrate:

  • 1st Level Registration, evidence of personal and professional development, and experience of working at senior Band 7 or equivalent.
  • Significant experience in specialist palliative care, leadership and change management, and working in the community.
  • Experienced non‑medical prescriber, with clinical skills qualification (or expectation to achieve)
  • Advanced Communication Skills course, and mentorship/teaching experience.

We’re looking for a senior clinical decision maker who can provide advice, clinical, operational and strategic leadership to the community specialist palliative care team.

The Role

  • Act as a senior clinical decision maker providing advice to the Clinical Nurse Specialists and other Healthcare professionals in relation to complex situations.
  • Provide clinical, operational and strategic leadership to the community specialist palliative care team.
  • Work across Organisational boundaries, providing advice and face to face clinical assessment over 7 days. This will include patients in Salford Royal at weekends and on Bank Holidays
  • Demonstrate and develop the skills required to practice at an advanced level including prescribing and clinical examination skills.
  • Act in accordance with the Nursing and Midwifery Council Code of Conduct.
  • Demonstrate the core competencies of ACNS in supportive and palliative care:

Requirements

Essential education/ experience:
