Hours

37.5 hours per week. Weekend work required. The service currently operates seven days per week, between 8am and 8pm.

Introduction

We are looking for a Healthcare Assistant to join our Hospice @ Home Team to assist with the delivery of holistic care to patients, family/carers with life limiting illness, within their own living environment.

The Role

You will work as part of a compassionate, multidisciplinary team, working in partnership with the patient, family & carers.

You will:

• Assess, plan and deliver holistic, person-centred care.
• Support patients and carers physically, emotionally and spiritually.
• Keep accurate records and communicate clearly with the wider care team.
• Use your knowledge of local services to ensure the best support for every family.
• Occasionally provide cross-site cover when needed.

The Person

You will be a caring, proactive and a true team player with a person-centred approach.

You will have:

• NVQ Level 2 in Care (or willingness to work towards it).
• A full driving licence and access to a business-insured vehicle.
• Experience in hospital or community care settings.
• Good communication skills and confidentiality awareness.
• An interest in specialist palliative care and commitment to compassionate practice.

The successful candidate will be subject to an Enhanced DBS Check, the cost of which is covered by the hospice.
