Job Summary

We are looking for an experienced and motivated Social Worker to join our Patient and Family Support Team, working across our Little Hulton and Heald Green sites to support people living with a life limiting illness, and those close to them.

The Role

As part of a compassionate, multidisciplinary team, you will provide specialist palliative social work interventions that place the individual at the centre of care. You will bring professional expertise, emotional insight, and a commitment to supporting what matters most to patients and families.

You will:

  • Deliver specialist social work assessments and interventions in both inpatient and community settings.
  • Provide compassionate, person centred support to patients, families and carers navigating complex emotional, practical and safeguarding needs.
  • Offer expert guidance to colleagues on social work practice, legislation and safeguarding.
  • Work collaboratively with nurses, doctors, counsellors, chaplaincy and other professionals to ensure coordinated, safe and effective care.
  • Build strong relationships with external agencies to advocate for individuals’ rights, wishes and outcomes.
  • Contribute to service development, quality improvement, education and innovation within the hospice.

Requirements

You will be a committed and compassionate Social Worker who brings both professional expertise and a genuine passion for supporting people.

You will have:

  • A recognised Social Work qualification (Masters/Degree/DipSW) and registration with Social Work England.
  • Significant post qualifying experience, ideally including work within palliative care, health, adult social care or another complex setting.
  • Strong interpersonal, communication and assessment skills, with the ability to work sensitively and collaboratively.
  • A sound understanding of relevant legislation, including the Mental Capacity Act, Care Act and safeguarding frameworks.
  • Confidence in managing complex information and maintaining accurate electronic records.
  • The ability to work effectively within a multidisciplinary team and use initiative to prioritise work.
  • A commitment to ongoing professional development in line with Social Work England requirements.

The Counsellor will:

Requirements

  • Awareness of the importance of self-care.
  • A knowledge of relevant legislation and guidance (e.g. Mental Capacity and Safeguarding).
  • A good standard of computer literacy.
  • Access to a vehicle to enable independent travel between hospice sites
  • A commitment to continuous professional development in accordance with the organisation’s Competency Framework.

The Counsellor will be able to demonstrate:

  • In-depth knowledge of bereavement care.
  • Experience of working with loss and bereavement in a counselling context.
  • Effective communication skills, both written and verbal.
  • Organisational and time management skills.
  • Understanding and commitment to working within the code of practice and ethical framework of the relevant professional body (BACP).
  • An awareness of the importance of self-care when working with loss.
  • The ability to work flexibly to meet the needs of the service.
